We are currently looking for therapists (LPC, ST, LCSW, APC, LMFT, CACI, CACII ), LPN/RN and CSI/paraprofessionals Job Responsibilities & Performance Standards:

Writes service plans that are clear, concise, complete, grammatically correct, timely, and properly coded.
8.
Monitors consumer progress and compliance with the service plan.
Writes timely progress notes that relate directly to the treatment outcomes described in the service plan.
9.
Assembles, analyzes, and interprets new/updated information about the consumer in order to reassess treatment needs.
13.
Establishes linkages and coordination of services with community agencies.
